Aim
To develop a pathway that ensures access to short break services is understandable to all and responds to the needs of children, young people and their families.
Objective
1. Listen to parents/carers and evaluate and learn from current practice.
2. Make recommendations to the Project Reference Group of ways to develop transparent pathways into short break services, which takes account of entitlement, assessment processes and the need to coordinate short break services.
Resources
Allocated through the pathfinder budget 2008/09 - not identified at present time but resources available for participation.
Membership of Group
Two or three parent representatives from different parts of the county whose children are accessing different short break services; one Team Manager, Social Workers, health representation.
